Education Maintenance Allowance Documents

There are a number of forms relating to EMA which are located in the Current Documents Section to the right:

• Bank Mandate (279kb pdf) - this form should be used by young adults to supply bank account details to the EMA Unit.  
• Pupil Self Certification Form 2009-10 (pdf 94kb) - this form should be used by EMA applicants for verifyinig self certified sickness absence
• Terms and Conditions for Students - School (pdf 139kb) - Highland Council EMA Programme terms and conditions which all awarded students of EMA must adhere to.  (This is attached to the award letter sent to the student and can also be obtained from the school) 
• Terms and Conditions for Students - Home Taught (pdf 139kb) - Highland Council EMA Programme terms and conditions which all awarded students of EMA must adhere to.  (This is attached to the award letter sent to the student) 
• Terms and Conditions - Learning Centres (pdf 139kb) - Highland Council EMA Programme terms and conditions which all awarded students of EMA must adhere to.  (This is attached to the award letter sent to the student and can also be obtained from the school) 
• EMA Payment Schedule 2009-10 (pdf 47kb) – School and Home Educated this is the EMA payment schedule for all awarded EMA students and gives information on when payments are to be paid and cut off dates.  
• EMA Payment Dates 2009-10 (50kb pdf) Students – Third Sector this is the EMA payment schedule for all awarded EMA students and gives information on when payments are to be paid and cut off dates.    
• Part C1 and C2 (509kb pdf) - this form should be used by parent/carer/s to verify social security benefits. 
• Part C IND (309kb pdf) - this form should be used by young adults who are living independantly from their parent/carer/s to verify social security benefits
