Vision For Transition

‘A Vision for Transition’ is an authority wide multi – agency project whose aim is to improve the transition process for all young people, with a focus on those with additional support needs and those at risk of not moving on to a positive post school destination. The Highland Psychological Service has been involved in this project in all aspects of its development.

It's MY Journey

It's MY Journey asked close to fifty young people about their experiences of transition planning and transition. The report of this consultation can be found  at Highland Children's Forum.

Tracking Study

A Tracking Study was undertaken with four young people with additional support needs to get an overview of the meeting process of transition for young people with additional support needs from secondary to adult life.

Questionnaire for Professionals

As the guide is a tool for professionals as well as children and young people it was important to find out what the professionals who support children and young people through transition thought would be important to improve transition. The questionnaire asked participants to rate various aspects of their organisation's transition proces and identify what could help them to move up the scale.

My Transition Guide

The Highland Council Transition Coordinator led a multi agency team in developing a guide for transition planning, 'My Transition Guide'. The Transition Guide acted on the recommendations of young people in 'It's MY Journey' and consolidated and built on existing good practice in Highland.

My Transition Guide comes in two booklets which can be found in the Current Documents section on this page. One Guide is written for Children and Young People and the other for Parents, Professionals and Partner Services.

It's MY Choice

It's MY Choice provides information for young people with additional support needs, their carers and professionals involved in their support about the process of transition from secondary school to adulthood. The Transition Coordinator worked with Children in the Highlands Information Point to update and enhance It's MY Choice.

Full and Summary Report of the Evaluation of the Pilot of the Transition Guide

The Transition Guide Pilot Evaluation was conducted to provide evidence as to whether the guide had achieved the indicators for success as detailed in the documents It’s MY Journey, The Tracking Study, Professionals Questionnaire and the Transition Guide itself.
The pilot was undertaken in partnership with eight secondary schools, four primary schools and six partner services. The full evaluation and summary evaluation can be read or downloaded from the Current Documents section on the right hand side of this page.
