Adults With Incapacity

Welcome to the Highland Adults with Incapacity Website.

This site is designed to be used by staff from Highland Council and the Highland Primary Care NHS Trust.

This site contains links to all the documents you will need when using the Act, including the Act itself, Codes of practice, Forms, Regulations, Procedures, and Training Materials.   Just click on the appropriate link and you can download the relevant document.

All the prescribed forms are here, and they can be downloaded and completed electronically or manually.  There are also exemplar Summary Applications. The forms most likely to be needed are AWI 1 (Incapacity Report), AWI 2 (MHO Guardianship Report) and AWI 3 (MHO Guardianship Renewal Report).
