Returning from hospital

Equipment and adaptations

If you are coming out of hospital and need equipment at home, the hospital-based Occupational Therapist, Physiotherapist or Community Nurse will help. They will make arrangements for the equipment to be delivered to your home.

If your home needs adapting, or is not suitable, they can contact the local Health and Social Care Team to assist you.

Your Occupational Therapist is responsible for getting your equipment collected once you no longer need it.

Housing options

If you feel your home is no longer suitable and you need to move, speak to your local Housing Team for help and advice on all your housing options. They can help if you are thinking about your future and how you will manage at home. Anyone can get advice, whether you own your home or rent.

Make sure to contact them as early as possible as this gives time to find a suitable solution. This can help avoid a crisis.

Contact them to arrange an appointment or speak to the hospital staff for help with arranging this.
