Self-Directed Support (SDS)

Support plan

With support from a parent or carer, the young person is encouraged to think about how they want to live their life and what they would like to achieve. These personal goals - also known as outcomes - are central to the support planning process and reflect what matters most to the young person and their family.

Identifying personal goals

Together with their parent or carer, the young person may explore:

  • what is currently working well in their life - and what is not
  • what is important to them and what they enjoy doing
  • what support they and their carer need to live the life they want
  • ideas for how to make their goals a reality
  • how to stay safe while working towards their goals
  • how they want to be involved in making decisions about their life

Reviewing the support plan

The young person, their parent or carer, and a support worker will meet at least once every 12 months to review the plan. This meeting is an opportunity to check how things are going, whether the agreed outcomes are being met, and to make any necessary updates to the plan.
