Review and Budget
The young person, their parent or carer, and a support worker will meet at least once every 12 months to review the plan. This meeting is an opportunity to check how things are going, whether the agreed outcomes are being met, and to make any necessary updates to the plan. This includes reassessing whether the child or young person still meets the eligibility criteria for support from the Disability Service.
Regardless of the option chosen, the budget must be used solely to achieve the outcomes outlined in the support plan.
