Help with housing when leaving hospital

If you are leaving hospital and your home may not suit your long-term needs, we can help.

This may be because:

  • your home does not meet your physical needs
  • your home does not meet your mental health needs
  • your occupational therapist has said your home is not suitable
  • rehousing has been recommended to support your long‑term health

We can help you explore your housing options and help with these.
